The Health Care Team is a provider of Physical, Occupational and Speech-Language Therapy Services and is a Certified Rehabilitation Agency. Our Team of health care professionals are all licensed an/or certified in their discipline, have many years of experience and are dedicated to quality patient care.

The Health Care Team provides a variety of quality care services, the most common being:

The Health Care Team works with the primary care Physician/Dentist to establish a treatment plan that best benefits the patient. The patient remains under the overall direction of their Physician or Dentist and are referred back every 30 days fro re-evaluation.

During an initial assessment, the physical therapist will perform a variety of tests which will determine the current situation and rehabilitation requirements of the patient. Once this assessment is completed, a physical therapy plan will be developed for the patient. The focus of this plan will be:

To improve and restore functionality after injury, surgery or disease;
To alleviate or significantly reduce pain;
To prevent further injury and disability or to slow the progression of conditions resulting from injury, surgery or disease;
To provide education on maximizing health, fitness and prevention; and
To improve patients quality of life by increasing independence and self-sufficiency.

The physical therapist will work side by side with the patient to ensure the goals of the physical therapy plan are achieved.
